5 Tips For Safe And Long-Term Weight Loss

Choosing lower-calorie foods and having smaller portions can help you lose weight.

Sustained long-term weight loss should be viewed as a lifelong commitment and in order for it to be effective, one has to make gradual permanent changes to their current lifestyle. Lifestyle changes often require time, adopting new skills, identifying current behaviours that have led to weight increase and replacing them with healthier habits. As behaviour modifications do not take place overnight, support from family members and friends can be beneficial. Generally, weight regain is more likely if there is rapid weight loss compared to slow and steady weight loss.

The best and safest way to lose weight is to reduce your energy intake whilst increasing your energy output, i.e. eat less energy and burn off more energy. It is the net energy intake (i.e. the energy consumed for the day minus the energy expended through physical activity and for body functions) that determines if we lose or gain weight.

Limiting high-calorie foods, choosing lower-calorie alternatives, and cutting down on your food portion sizes are some ways to lose weight.


Strategies for safe and effective weight loss

1. Eat regular meals
Avoid skipping meals. Having regular meals give you the energy you need to get through the working day. Irregular meals lead to snacking and make you lose track of portion sizes.

2. Limit snacks
Snacking on deep fried, oily or sweet foods (e.g. cakes, kuehs, pastries, etc) add extra calories to your diet. If you experience hunger pangs, choose lower-calorie options like fresh fruit, low-fat yogurt and or tuna/sardine/low fat cheese sandwiches made from wholemeal bread (don’t forget to include veggies like cucumber and tomato slices!), as they are more filling and nutrient-dense.

3. Reduce fat
Cutting down on fat intake is an effective way to reduce energy intake and lose weight, as fats are a concentrated source of calories.

  • If you cook at home, remove visible fats from food before cooking.
  • Choose low-fat cooking methods (e.g. steaming, grilling, baking, roasting, boiling or pan- frying with lesser oil) to keep your dishes lower in fat and energy.
  • Avoid oily and deep-fried foods and dishes cooked with coconut milk. Always skim off any visible oil in your soups and dishes with gravy before eating.

4. Reduce sugar

Sugar gives energy but has little valuable nutrients. To lose weight, limit candies, desserts, cakes, kuehs and sweet pastries as they contain a lot of added sugars. You should also watch your beverages – choose plain water over sweetened drinks and juices. Drinking too many sweetened drinks and juices will put you at risk of obesity and tooth decay.

5. Fill up on fibre

Snoring(due to airway obstruction), restless sleep and sleepiness are the most common symptoms shown in Sleep Apnoea Syndrome (SAS).During a seven hour sleep, if breathing is stopped more than 30 times and each time is above 10 seconds this is a clear sign of Sleep Apnoea Syndrome. During sleep apnoea, a person will experience an increase in Carbon Dioxide(CO2) in the body. Chemoreceptors are receptors which respond to any changes in CO2 or Oxygen(O2). There are two main types of chemoreceptors, central and peripheral and both responds to an increase in Carbon Dioxide. As a result, this causes the person to wake up to resume eupnoea (normal respiration) and allows the person to fall asleep.
Diagnosis of sleep apnoea includes waking up during sleep, thirst, daytime sleepiness, fatigue, learning/memory difficulties and sexual dysfunction.
Dyssomonia which refers to sleep disordered breathing leads to an increase risk in cardiovascular disorder, stroke, hypertension, diabetes and etc.
Obesity has a TREMENDOUS link with SAS. Poor sleep causes people to eat more, which in turn cause obesity. There is a relation between sleep deprivation and hunger and satiety hormones (from the ventromedial nucleus of hypothalamus), eg. Leptin and Ghrelin. Individuals with sleep aponea often have hypertension, high fasting glucose (sugar), and high cholesterol, all of which can be made worse with sleep deprivation. This leads to a vicious cycle. Therefore, an effective and long term measure to prevent SAS is to prevent obesity (BMI>30).

The process of acupuncture (placing of needles into the acupuncture points which fall along the meridians) corresponds with the location of disharmony of the qi to balance and allow it to flow freely again, thereby alleviating the symptomatic problems.

How does obesity affect my health?

Obesity is a growing epidemic that is affecting billions of people around the world. According to the National Health Survey (2004), 25.6% of Singaporeans are overweight (BMI: 25.0kg/m2 – 29.9kg/m2) and 6.8% are obese (BMI > 30.0kg/m2).

It is well established that obesity increases the risk of developing chronic diseases, including Type II Diabetes Mellitus, cardiovascular diseases and several types of cancer. Individuals who are obese are also at higher risk of developing musculoskeletal disorders1 such as osteoarthritis2 of weight-bearing joints, chronic kidney disease and obstructive sleep apnoea. These obesity-related co-morbidities increases the individual’s mortality, decreases the functional capacity and quality of life.

How can I treat obesity?

A modest weight loss of about 5 - 10 per cent of the individual’s current body weight has been shown to be capable of reducing cardiovascular risk factors. The weight loss helps to decrease blood pressure in hypertensive and pre-hypertensive patients; improve insulin sensitivity and glucose tolerance in patients with impaired fasting glucose or Type II Diabetes Mellitus; increase high-density lipoprotein (“good” cholesterol”); and decrease total cholesterol and triglycerides in hyper-lipidemic patients.

Despite the increased availability of medications and surgical interventions that promises to treat obesity, lifestyle modification is key to obesity management. Lifestyle modification has been shown to be capable of preventing the development of chronic diseases such as Type II Diabetes in obese and high-risk patients.

A comprehensive and effective obesity management focuses on health gains but not weight loss via lifestyle modification. It emphasises on healthy changes to the individual’s dietary habits, increase in daily physical activities, structured exercise programme and behavioural modification (such as motivation techniques, stress management).

1. Improve diet

Dietary changes have also been shown to be effective in lowering cardiovascular risk factors, such as blood lipid levels. Epidemiology studies have also shown a relationship between dietary habits and chronic diseases, suggesting strongly that dietary changes will have favourable effects on disease prevention. Studies have shown that people who are most successful in losing weight and maintaining the weight loss eat a light but healthy breakfast, have a calorie-reduced diet that is low in fat instead of following fad diets and have a high fibre intake.

2. Increase daily physical activities

Individuals who are obese but physically fit have lower mortality and morbidity compared to those who are skinny but physically unfit. Physical fitness is now established as an indicator of mortality, independent of the individual’s weight status. Adults should aim to achieve at least 10,000 steps daily to maintain healthy weight. Sedentary individuals aiming to reduce weight should start by achieving 2000 steps above the current number of steps taken. Individuals are advised to have at least 30 minutes of moderate intensity aerobic exercises (eg: brisk walking, cycling, swimming, jogging) for most, if not all days of the week to maintain healthy weight.

3. Adopt positive behaviours

Behavioural modification is one of the most crucial components of all lifestyle modification interventions. Many studies have shown the effectiveness of behavioural modifications via counselling to adopt healthy changes in dietary habits and increase amount of physical activities. Such counselling can done via the telephone, internet or in-person, all of which have been shown to be effective. Begin by setting realistic and achievable goals – this can be done with a physician or dietitian specialising in obesity. Identify causes of weight gain or reasons for unsuccessful weight loss by conducting a thorough examination of current lifestyle and medical history. Develop specific strategies to overcome these barriers and set achievable goals. Do not be overly ambitious - establish small objectives (eg: “I will reduce time spent watching television by 30 minutes, 3 times a week and go for a brisk walk in the park instead”). Provide non-food incentives as rewards when goals are achieved.

All these lifestyle modifications are best done with your specialist physician, dietitian or trained counsellor. Follow-ups are important to maintain weight loss, set new goals, improve current diet and exercise habits further and new lifestyle habits.

Can Meal Replacement Help You Lose Weight? By Singapore General Hospital

Study suggests that replacing one meal with a weightloss shake or bar may be more effective than relying solely on a low-calorie diet

Replacing one meal in a low-calorie meal plan with a weightloss shake may prove effective.Eating less rich foods may not lead to a smaller waistline. Instead, replacing one meal in a low-calorie meal plan with a weight-loss shake or bar may be more effective than merely following a low-calorie diet alone, a study by a group of Singapore General Hospital (SGH) dietitians found.

People who are overweight or obese are put on a weight-management programme that includes a low-calorie diet, or such a diet combined with a meal replacement.

“But the effectiveness of meal replacements has not been studied in depth, so we wanted to find out just how useful they are,” said Ms Meina Armanda, a member of the team that included Ms Tan Ai Shan and Mr Tan Lee Boo, all from the Department of Dietetics.

Cutting calories

A low-calorie diet works on the principle that if a person consumes fewer calories than he uses up, he will lose weight. The team looked at several studies done to evaluate the effectiveness of a partial meal replacement diet against the standard low-calorie one.

“The results showed that (the people who followed) partial meal replacements had a greater overall weight loss compared with those who relied on a reducedcalorie diet alone,” said Ms Armanda. They found that people on the partial meal replacement plan lost a significant 2.7kg, and 1.6kg more than those on the reduced-calorie diet after three and six months respectively.

There were limitations to the review as not all the trials that the group studied were done in a controlled environment, but the results were promising enough for the team to consider further study. “Going further, we can try to isolate the variables and factors,” she said.

Replacing a meal

Rather than skip meals to lose weight, have a meal replacement bar instead.Meal replacement shakes or bars are nutritionally balanced products that people trying to lose weight can take in place of some meals. In a diet programme monitored closely by a dietitian, taking appropriate meal replacement products can be useful, said Ms Armanda. “Instead of skipping meals, meal replacements can help people lose weight as they are nutritionally balanced, convenient and easily accessible.”

A diet that includes a partial meal replacement can also help people comply with their diet by reducing hunger pangs and the effects of the so-called yo-yo diet syndrome, which happens when the body responds to an excessively low-calorie intake during a “crash diet” by adjusting the rate at which it burns calories to support the body’s various daily functions. That slower rate of metabolism continues even when the person returns to normal eating, which can lead to weight gain.

“The yo-yo effect happens because crash dieting prompts the body to store fat at a faster rate once a person starts eating normally. This is the body’s protective instinct trying to store reserves for future periods of deprivation,” Ms Armanda said.

Instead of going on a crash diet after binge eating, people should try to lose weight by making sustainable changes to their lifestyles and eating habits, she said.

The SGH team’s study won the first prize in the Best Oral (Allied Health Evidence- Based Medicine) category at the SGH Annual Scientific Meeting in 2011, a conference showcasing the research and studies undertaken by doctors and other healthcare staff at SGH Campus, including Duke-NUS Graduate Medical School.
